What is a DOT SAP Evaluation?

A SAP evaluation is a federally mandated process for employees in safety-sensitive positions who have violated DOT drug or alcohol regulations.

The SAP:

  • Evaluates the violation

  • Determines required education or treatment

  • Reviews completion documentation

  • Determines eligibility for Return-to-Duty (RTD) testing

The SAP does not decide employment status or conduct drug testing.

This applies to workers regulated by:

  • Federal Motor Carrier Safety Administration (FMCSA)

  • Federal Aviation Administration (FAA)

  • Federal Railroad Administration (FRA)

  • Federal Transit Administration (FTA)

  • Pipeline and Hazardous Materials Safety Administration (PHMSA)

  • United States Coast Guard (USCG)

What the SAP Does — and Does Not Do

SAP Responsibilities:

  • Conduct SAP evaluations

  • Determine required education or treatment

  • Review completion documentation

  • Determine eligibility for RTD testing

  • Enter required dates into the DOT Clearinghouse (when applicable)

The SAP Does Not

  • Schedule or pay for treatment services

  • Control employer timelines or testing availability

  • Submit negative Return-to-Duty test results

  • Change Clearinghouse status to “Not Prohibited”

  • Resolve employer or third-party administrator delays

SAP Fees and payment information

Private-Pay SAP Services

  • Flat fee: $500

  • Payment is due at the time of the initial SAP evaluation.

The SAP fee includes:

  • Initial DOT SAP evaluation

  • Written SAP recommendations

  • Case management to verify DOT-compliant referral sources

  • Review of completion documentation

  • Follow-up SAP evaluation

  • RTD eligibility determination

  • Required Clearinghouse reporting (when applicable)

  • Case closure documentation

The SAP fee is non-refundable and applies regardless of outcome or completion of recommended services.

 

The SAP fee covers professional evaluation and required reporting. It does not include ongoing administrative advocacy with employers, third-party administrators, or treatment programs beyond what is required under federal regulations.

DOT Clearinghouse reporting requirements

For FMCSA-regulated drivers:

  • The SAP enters the date of the initial SAP evaluation and the date the driver is eligible for RTD testing.

  • Employers enter the negative RTD test result and completion of follow-up testing.

The SAP cannot change a driver’s Clearinghouse status to “Not Prohibited.” Only the employer can do so after submitting a negative RTD test result.

Clearinghouse records remain visible for five years from the date of the violation or until follow-up testing is completed, whichever is later.

Important Timing Information

  • SAP evaluations are valid for six months.

  • If recommended services are not started within six months, a new SAP evaluation is required.
